The twice-weekly treatment of a Yemeni woman who emigrated alone as a young adult to the United States will be discussed. The presentation will explore the impact of sadomasochistic dynamics in the patient’s family of origin on her adult relationships as well as the treatment. The patient’s disavowed rage is acted out through masochistic tendencies such as social media “stalking” and by placing herself in dangerous circumstances, particularly with men. She also has a tendency to act sadistically in a desire to exact revenge in an effort to avoid the experience of vulnerability. The challenge of maintaining a neutral stance, managing worry and helping to reduce defenses against vulnerability for patients presenting with these types of dynamics will be explored.

Learning Objectives:

Participants will be able to:

1) Recognize how sadomasochistic dynamics with caregivers can impact adult relationships.

2) Increase their understanding of how to support patients through sadomasochistic conflicts.

3) Become more aware of the impact of culture, immigration status and the current political climate on our patients and treatment.

Training opportunities, including flexible scheduling and financial options to fit individual needs, will be described following the presentation.  Refreshments will be served.

MITPP, offers three certificate programs: a three year program in Psychoanalytic Psychotherapy with Adults, a two year program in Child and Adolescent Psychotherapy, and a program leading to the New York State license in Psychoanalysis. Matriculants see patients at The Metropolitan Center for Mental Health, a licensed OMH facility. Social workers’ clinical hours qualify toward the LCSW.  Candidates may be offered the opportunity to be paid for clinical work if they qualify to see 14 patient hours weekly.  Truly affordable training: Low tuition of $1,200 per semester includes both individual and group supervision and up to three courses.  Courses are also offered on a non-matriculated basis and, upon matriculation, are applied toward a certificate. Candidates begin seeing patients as soon as they are processed.
